To communicate Privacy Regulations to our patients and families we have developed a "Notice of Privacy Practices". It outlines patient privacy rights and related CHHS responsibilities.
CHHS is required to provide a copy of this notice to all patients and research participants.

The notice is also posted in public ares of our facilities and on our web site www.chw.org. |
